What is Inform Alberta?
The Grande Prairie Volunteer Services Bureau is working with the City of Grande Prairie to develop an online database of services in our City. Inform Alberta is a province wide database of community services and is linked to 211 Community Information services. While 211 is currently unavailable in our region, we aspire to create a thorough directory for our region so that the 211 system can be activated here.
What information is gathered?
The Grande Prairie Volunteer Services Bureau requires information about the organization, its services, and additional locations, in order to develop complete and accurate profiles on the Inform Alberta website. If you work with a non-profit organization or community service agency, please download, complete, and return the documents below to:

Instructions for Completion of the Organization, Service, and Location Profiles
- Complete the Organization Profile first. This form outlines the basic information about your organization such as your official name, mandate, address, etc.
- Complete a Service Profile for each service. This form outlines the information about each service / program offered by the organization. If your agency provides more than one service, please record them separately, but do not exceed three service profiles.
- Complete the Additional Location Information sheet. A location is the actual building in which you are located. Sometimes there may be multiple services offered in one location or sometimes a services is operated out of multiple locations.
- Send the completed package (Organization, Service, and Location Profiles) to the Grande Prairie Volunteer Services Bureau using the contact information listed above.

What is 211?
211 is an easy to remember telephone number that, when dialed, will give the caller access to information on community services. 211 call centres employ certified Community Information and Referral Specialists to ensure the highest quality of information provided to the caller. With your help, we can make 211 a reality for our community.
